This equipment contained hazardous energy sources such as, but not limited to, 208VAC - 3 phase electricity, high temperature steam lines, mechanical moving parts, and compressed air.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    a) For the facility at 1815 Cypress Lake Dr., Orlando, FL 32837 as observed on or about 11/12/2014, an electric Toyota forklift that was operated at the facility had worn/damaged tires. Rubber chunks were missing from the rear tires resulting in over 1 inch wide sections of the tires being removed, exposing the rim.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv)(a):    For the facility at 1815 Cypress Lake Dr., Orlando, FL 32837, as observed on or about 11/12/2014, flexible extension cords were used as permanent wiring, run through ceilings, attached to walls, and run underneath doors in the following areas:    a) In the kitchen area extension cords were run through ceilings, attached to the building, and used in place of fixed wiring.    b) On the main production floor extension cords were run through the ceiling and used in place of fixed wiring.    c) In the maintenance shop, extension cords were run under doors and used in place of fixed wiring.
