29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about November 10, 2014, employees working without head protection were exposed to struck-by hazards from falling oven roof deck panels when Innovative Mechanical Group, Inc. did not ensure head protection was used while a support column was being plumbed-up. Abatement documentation is required for this item in accordance with the requirements of 20 CFR 1903.19(d).

29 CFR 1926.754(d)(2): Plumbing-up equipment was not in place and properly installed before the structure was loaded with construction material such as loads of joists, bundles of decking or bundles of bridging. On or about November 10, 2014, employees plumbing-up a corner column were exposed to struck-by hazards from falling oven panels when Innovative Mechanical Group, Inc. did not ensure shims were properly installed before oven roof decking panels were loaded onto cross beams. Abatement documentation is required for this item in accordance with the requirements of 20 CFR 1903.19(d).
