29 CFR 1910.303(a): The conductors and/or equipment required or permitted by this subpart were not approved:  Employees were exposed to a 110 VAC electrical shock hazard.  The remote on/off switch taped to the inlet hose of the wall mounted vacuum was job made and was not listed, labeled, or approved by a nationally recognized testing laboratory or the equipment manufacturer.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Employees were exposed to a 110 VAC electrical shock hazard.  The live conductors providing power to the job made, remote on/off switch on the wall mounted vacuum inlet hose were not protected from employee contact.   29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

The wall mounted vacuum at the LB table was provided power through 2 extension cords.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
