Location: Copper Square Road, Building 62, Bethel, CT 06801 A) The guardrail system along the 7 foot long open sided 2nd floor hallway was not equipped with a midrail. B) The midrail along the 14 foot long open sided 2nd floor hallway was installed 11 inches above the floor.
