29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates, mud sills or other adequate firm foundations:  (a). At Lot 340 where exterior stucco was being conducted, employees were exposed to fall hazards in that the supported scaffold that they were working from did not have mud sills placed under the support legs while on sandy soil, on or about November 12, 2014.

29 CFR 1926.451(e)(1):  Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  (a). At Lot 340 where a supported scaffold was being used employees were being exposed to fall hazards in that, no ladder  was provided for access and egress, on or about November 12, 2014.
