29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) Cameron International Corporation at 8750 6th Street, Wellington, CO 80549:  On or before December 2nd, 2014, an employee using pneumatic tools to perform his duties as a valve technician was exposed to noise in excess of the action level.  The employer did not implement an effective hearing conservation program.    One valve technician in the central bay area , was exposed to continuous noise at 70.5% of the permissible exposure limit or an equivalent sound level of approximately 87.5 dBA during the 446 minute sampling period on December 2nd, 2014; exposure calculations included a zero increment for the 34 minutes not sampled.

29 CFR 1910.134(g)(1)(i): The employer permitted respirators with tight-fitting facepieces to be worn by employees who have facial hair that came between the sealing surface of the facepiece and the face or interfered with valve function:    a) Cameron International Corporation at 8750 6th Street, Wellington, CO 80549:  On or before December 2nd, 2014, an employee was observed performing spray painting activities while wearing a tight-fitting half mask respirator.  The employee was exposed to potential health hazards from the paint vapors as the employee had substantial facial hair which was present at the sealing surface of the respirator.

29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace:    a) Cameron International Corporation at 8750 6th Street, Wellington, CO 80549:  On or before December 2nd, 2014, employees performing spay painting activities were required to wear half mask respirators.  Employees were exposed to potential hazards from the misuse of respirators as the employer had not provided employees with effective training prior to requiring respirators to be worn in the workplace.

29 CFR 1910.107(c)(6): Electrical wiring, motors, and other equipment outside of but within twenty (20) feet of any spray area, and not separated therefrom by partitions, produced sparks under normal operating conditions and did not otherwise conform to the provisions of subpart S of this part for Class I, Division 2 Hazardous Locations:  a) Cameron International Corporation at 8750 6th Street, Wellington, CO 80549:  On or before December 12th, 2014, employees were exposed to a potential fire hazard from the operation of the small spray booth as the light switch to an overhead light and the controls to the spray booth were within 20 feet of the spray area.
