On or about Thursday, November 13, 2014 at 20-12 233rd Street, Bellerose, NY 11427  Employees were observed working on a residential rooftop approximately 25 feet above the lower level installing plumbing equipment and a chimney cap without a means of fall protection in place.  ABATEMENT CERTIFICATION IS NOT REQUIRED

On or about Thursday, November 13, 2014 at 80-12 233rd Street, Bellerose, NY 11427  The employer failed to develop and provide employees that are required to work at elevated working levels a fall protection training program.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
