29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.   a.     On or about November 14, 2014, in the main building, employees of Kann Enterprises, INC., were required to operate forklifts and move pallets of tires on a daily basis. The north / central exit route was blocked by tires and pallets.  Employees were thereby exposed to the hazards associated with blocked exits.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998]    a.     On or about November 14, 2014, in the warehouse area, employees of Kann Enterprises, INC., were required to add water to the forklift batteries that contained sulfuric acid, a corrosive liquid. The employees were not provided with suitable facilities for quick drenching or flushing of eyes. The facility had a portable eye wash station that had no water in it. Employees were thereby exposed to the hazards associated with chemical contact.

On or about November 14, 2014, in the shipping / receiving area, employees of Kann Enterprises, INC. were required to operate forklifts and move pallets of tires on a daily basis.  Emergency exit signs with exposed wires were hanging in the racks and aisle ways. Employees were thereby exposed to electrocution hazards.   b.     On or about November 14, 2014, in the shipping / receiving area, employees of Kann Enterprises, INC. were required to operate forklifts and move pallets of tires on a daily basis.  Adjustable dock light fixtures that were present and / or in use (live) had exposed wire. Employees were thereby exposed to electrocution hazards.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a.     On or about November 14, 2014, at the above located workplace, employees of Kann Enterprises, INC., were required to work near and around a 480 volt power distribution box. Sufficient access and working space was not provided and maintained about the power distribution box. The area around the power distribution box was cluttered with packaging material and pallet appendages. Employees were thereby exposed to electrical hazards.  b.     On or about November 14, 2014, at the above located workplace, employees of Kann Enterprises, INC., were required to work near and around battery chargers. Sufficient access and working space was not provided and maintained about the battery chargers. The area around the battery charges was cluttered with packaging material, wooden slats (with nails), and other debris. Employees were thereby exposed to electrical hazards.

On or about November 14, 2014, in the shipping / receiving area, employees of Kann Enterprises, INC., were required to operate forklifts and move pallets of tires. Adjustable dock lighting apparatus' were present and powered by conductors not properly protected from abrasion in accordance with OSHA standards, thereby exposing employees to the hazards associated with electricity (electrocution, fire, and arc flash).

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.   a.      On or about November 14, 2014, in the shipping and receiving area, employees of Kann Enterprises, Inc., were required to operate forklifts and move pallets of tires daily.  A live junction box (120 volt) was not provided with a cover.  Employees were thereby exposed to the hazards associated with energized electrical parts.

29 CFR 1910.157(c)(4): The employer shall assure that portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use.  Warehouse - On or about November 14, 2014, employees of Kann Enterprises, INC., were required to operate forklifts and move pallets of tires on a daily basis.  The employer did not ensure that all portable fire extinguishers were kept in their designated places at all times. Approximately five fire extinguishers in the east corridor (receiving dock) of the warehouse were not present at their designated location.
