29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees. a. At the work site at 169-171 Broadway, Jackson, Ohio, employees worked over six feet from the floor while removing floors, joists, ceilings and roofing on a two story building. The employer did not provide employees with fall protection, eye protection, head protection and/or hand protection.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. In October 2014, at the work site located at 169-171 Broadway, Jackson, Ohio, an employee was injured when the second floor joists on which he was standing collapsed and he fell approximately ten feet to the concrete floor. The employer did not ensure that the unprotected edges of the work surface were guarded to prevent employees from falling ten feet to the floor below.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed. a. In October 2014, at the site located at 169-171 Broadway Street, Jackson, Ohio, the employer did not have a written engineering survey before allowing employees to begin demolition work on a two-story brick building that was being gutted.

29 CFR 1926.854(b): No wall section, which is more than one story in height, shall be permitted to stand alone without lateral bracing, unless such wall was originally designed and constructed to stand without such lateral support, and is in a condition safe enough to be self-supporting. All walls shall be left in a stable condition at the end of each shift. a. In November 2014, at the work site located at 169-171 Broadway, Jackson, Ohio, the employer had employees remove all interior structures and debris from the building, leaving only the brick exterior walls and a brick wall down the center of the building. The employer did not ensure the walls were left in a stable condition while employees created a storage area for files and office equipment.
