29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used: a) 406 Centre Street, South Orange, NJ Employees were conducting carpentry work from a ladder jack scaffold at heights of approximately 25 feet above ground level. The employer did not provide a point of access to the scaffold platform, exposing employee(s) to fall hazards. Condition observed on or about 11/13/2014.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) 406 Centre Street, South Orange, NJ Employees were conducting carpentry work on ladder jack scaffold at a height of approximately 25 feet above ground level. The employer did not ensure the use of fall protection, thereby exposing employee(s) to fall hazards. Condition observed on or about 11/13/2014.
