29 CFR 1926.351(a)(2): Current carrying parts of manual electrode holder(s) passing through the gripping portion or the outer surfaces of the jaws were not fully insulated against the maximum voltage encountered to ground: a) BEQ Bldg. 3601 Renovation Project, Wing "C", Ground Level, Joint Expeditionary Base Little Creek, VA: On or about 11/14/14, the insulators surrounding the outer surface of the grip handles of two electrode holders were missing insulation, thus exposing employee(s) to an electrical shock hazard. No abatement certification or documentation is required.

29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected: a) BEQ Bldg. 3601 Renovation Project, Wing "C", Ground Level, Joint Expeditionary Base Little Creek Virginia Beach, VA: On or about 11/14/14, employees of Barco Steel Construction were performing operations with a welding cable that contained cuts in the insulation. The cuts in the welding cable were observed approximately 31 and 61 inches from the electrode holder. Both cuts in the insulations were wrapped with electrical tape to cover the exposed internal conductor wires, thus exposing employee(s) to possible burn and electrical shock hazards No abatement certification or documentation required.
