29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, or when such equipment was required to approach the edge of an excavation, and the operator did not have a clear and direct view of the edge of the excavation. a) Huber (Maxwell) Shaft, 101 South Main Street, Ashley, PA - Employees drove and/or parked hydraulic excavators adjacent to a one thousand twenty three-foot (1023') deep mine shaft (excavation) surrounded by stacked construction vehicle tires that prevented a clear and direct view of the edge of the mine shaft, exposing employees to vehicles rolling over the edge and impact hazards, as observed on November 13, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
