29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Location: Maintenance shop  On or about 11/11/14, the employer did not develop and utilize energy control procedures to protect employees from the unexpected start up and amputation hazards from performing maintenance and servicing such as but not limited to on the rear load trucks while changing the main piston, replacing gaskets inside the hopper area, welding in new scraper plates, and replacing hydraulic lines.

Located at 210 Holt Road, North Andover, MA:  On or about 11/12/14, the employer did not record the following workplace injury and illness on the OSHA form 300 or equivalent for calendar year 2014;  a)    On or about 10/31/14, a mechanic while performing maintenance on the rear differential of a truck crushed and broke the left index finger when it slipped and fell.  The employee returned back to work with restricted duty and the employer did not record the injury on the OSHA form 300.  Abatement Certification and Documentation is required
