Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.  a.  On or about November 14, 2014 - at the above addressed jobsite, an employee was performing masonry/tuckpointing operations from a tubular welded frame scaffold and a two point suspended scaffold that was not designed by a qualified person, thereby exposing the employee to fall hazards.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.  a. On or about November 14, 2014 - at the above addressed jobsite, an employee was performing masonry/tuckpointing operations from a tubular welded frame scaffold outrigger/outlook bracket approximately eighteen (18) feet above the ground.  The scaffold platform was not at least 18 inches wide, thereby exposing the employee to the hazards associated with falls.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a.  On or about November 14, 2014 - at the above addressed jobsite, an employee conducting masonry/tuckpointing operations from a tubular welded frame scaffold, approximately eighteen (18) feet above the ground, was not provided with an access ladder or equivalent safe access to the scaffold plank, thereby exposing the employee to falls.
