For the facility at 104 Commerce St., Lake Mary, FL 32746, as observed on or about 11/14/2014, employees were exposed to the potential for skin burns and eye injuries from the usage of class IV laser systems, in that: Class IV lasers were used with their beams unguarded; a laser hazard assessment had not been developed; nominal hazard zones/boundaries for safe operation had not been determined; and employees who operated lasers were not adequately trained on the hazards present and were not aware of the laser's hazard class.
