29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Camping World - Warehouse: The Caterpillar Forklift Model #GC15, Serial #2EMO2975 had an attachment on the right fork that was used to haul RV camper trucks, the fork has a hole to screw the attachment. The employer did not provide the forklift manufacturer's written approval, occurred on or about 11/06/2014.
