29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a. Roof- Employees not protected from a trip hazard where adequate access was not provided to get over piping extending 36 inches above the roof surface. Employees were using a step ladder to get on top of the piping and then climbing down on the other side of the piping.
