29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    a)  Quality Sanitation Service - Tenderizer machine:  Where it was required, an energy isolating device had not been operated in a manner that isolated the machine from its energy source. An employee was injured while cleaning a machine while it was ON.  The employee got his hand caught in the point of operation.  Occurred on or about 11/15/2014.

Safeguarding workers from exposure to the machinery hazard(s) was not provided as required.  Employees were exposed to the tenderizer machine's point of operation while performing cleaning and sanitation operations.  Occurred on or about 11/15/2014
