29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. a. Warehouse - On or about November 17, 2014, in the warehouse, employees were required to operate powered industrial vehicles. The aisle ways in the shipping area of the warehouse were not kept clear (blocked by overstock). Employees were thereby exposed to the hazards associated with blocked aisles and passageways.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. Production Area - On or about November7, 2014, employees were required to operate the Republic Services Trash Compactor, Make SP, Model CP-2101, on a daily basis. The point of operation (an open pit) was not properly guarded, thereby exposing employees to the hazards associated with ungrounded open pits.

On or about November 17, 2014, at the above located workplace, employees were required to work in and around a 480 volt power distribution box which had multiple carts stored in front of it. c. On or about November 17, 2014, at the above located workplace, employees were required to work in and around a 480 volt power distribution box which had multiple pallets stored in front of it. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.
