29 CFR 1910.23(b)(1)(i): Every wall opening from which there is a drop of more than 4 feet was not guarded by one of the following: Rail, roller, picket fence, half door, or equivalent barrier.  On or about 11/4/14, in the production area at production platforms, the employer did not ensure openings around equipment were protected or covered. Openings measured 3 feet by 2.6 feet.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle.  On or about 11/4/2014, at the Buford facility, employees were exposed to equipment that was not equipped with guards to protect employees from the point of operations. Instances included:  1 - Craftsman Drill Press (Serial #178831)  2  - Feeder auger located on production platforms  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On 11/4/14, at the Buford facility, employees working near and around where welding operations occurred were not protected with welding screens or shields. 29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information.  On 11/4/14, at the Buford site, the employer did not ensure employees were trained in the Hazard Communication Program that included information required under the Global Harmonization System and new information pertaining to that program. Employees used chemicals including: Ecomass Compound 4703BI62, Nycal, MP1250, Acrawax C Lubricant, Songnox 1010, & Songnox 1098.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation.  On or about 11/4/14, at the Buford job site, the employer did not ensure injuries recorded were completed in the detail as required by the regulation. i.e. Column F descriptions were not complete and columns G-J were not properly marked for each injury listed on the log.
