On or about November 17, 2014 and times prior there to, at the workplace located at or near 7001 Hillwood Rd. Little Rock, AR, employees performed trenching work without adequate cave-in protection. This condition exposed employees to crushing hazards.

Little Rock, AR, employees worked in a trench approx. 8 ft. in depth that was not inspected by a competent person. This condition exposed employees to cave-in hazards.
