On or about October 9, 2014, on the roof, an employee using a DeWalt cut-off saw while cutting and installing ornamental brick was not wearing safety glasses, exposing him to eye injuries.

On or about October 9, 2014, employees operating two DeWalt cut-off saw that was missing the lower blade guard, exposing them to flying debris while cutting ornamental decorative brick.

29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):  a.  On or about October 9, 2014, in front entrance and above the garage, the tresses scaffold system was missing planks and employees were using the top of the scaffold to access and egress the roof while conducting ornamental brick work.
