29 CFR 1926.20(b)(2):     Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On 11/12/2014, the general contractor, sub-contractor, and two sub-contractor employees were working on the roof of a commercial building located at 11008 Schreckengast Road, Hickory Grove MO.  Roof Medics LLC, the general contractor and controlling employer, did not conduct an adequate inspection of the job site, materials, and equipment.  The following hazardous conditions existed:    a) Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.    b) Employees were using a ladder improperly to access the roof.  The ladder was not extended to three feet beyond the landing or edge of the roof.
