Bo Builders Inc. worksite located at 7120 Lighthouse Way, Perrysburg, Ohio: On or about November 12, 2014, the employer did not ensure the platform being supported by the JCB 550-170 rough terrain lift did not extend more than 10 inches past the wheel base. The 24.00 foot platform extended past the wheel base approximately 8.00 feet on both sides.

The unsecured platform was raised to a height of approximately 30.00 feet with an employee on the platform.

The exposed employees were installing trusses on the roof of the three story structure. Bo Builders Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501, which was contained in OSHA inspection number 912813, citation number 1, item number 1 and was affirmed as a final order on August 7, 2013, with respect to a workplace located at Rio Grande, Ohio.
