29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:       A. All Renovations Company worksite located at 12 Hidden Valley Dr. (Bldg. 4), Toledo, Ohio: On or about November 4, 2014, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     A. All Renovations Company worksite located at 12 Hidden Valley Dr. (Bldg. 4), Toledo, Ohio: On or about November 4, 2014, the employer did not ensure employees were protected from falls while applying new roofing material to a low sloped multi-family structure. Employees engaged in this activity were exposed to a fall distance of approximately 18.00 feet to the lowest level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    A. All Renovations Company worksite located at 12 Hidden Valley Dr. (Bldg. 4), Toledo, Ohio: On or about November 4, 2014, the employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.
