29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area:  On or about November 18, 2014, employee operating the polystyrene extruder had not been trained on the occupational health hazards of polystyrene or styrene.  Gar Plastics, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1) which was contained in OSHA inspection number 965263, citation number 1, item number 6 and was affirmed as a final order on September 25, 2014, with respect to a workplace located at 240 North Birdsey Street in Columbus WI.
