On or about 11/13/14, at site 102-37/55 Queens Blvd. Forest Hills, NY Employee was cutting metal frame with a Makita 241 4B cut-off machine without a guard. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
