29 CFR 1904.39(a): The employer failed to report to OSHA a work-related death within eight (8) hours after the death of the employee.  (a) A work-related fatality occurred on November 13, 2014, at the Mud Springs Timber Sale in the Targhee National Forest.  OSHA was notified by phone almost 24 hours later.
