29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a). At Bldg. #22 of the Mattamy townhome development an employee working from a stepladder on the second floor balcony was exposed to a fall hazard, in that, no guardrails were provided to protect him from a 10-foot fall to the ground below, on or about November 18, 2014.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  At Bldg. #22 of the Mattamy townhome development employees installing roof decking were exposed to fall hazards, in that, no alternative or conventional fall protection was being used to protect them from a 18-foot fall from the eave of the roof to the ground below, on or about November 18, 2014.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:  (a) At Bldg. #22, where an employee using an 8-foot stepladder was exposed to a 10-foot fall hazard, in that, the spreader bar was not completely opened  and locked, on or about November 18, 2014.
