29 CFR 1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  a) Production areas - Sanitation employees who used corrosive chemical products including, but not limited to Quip Laboratories, Inc.s Chlordet (potassium hydroxide) and Formula 3950 (potassium hydroxide) were not required to wear chemical splash goggles and full face shields. On or about 11/18/14.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  a) Production areas - Sanitation employees were not informed that spaces including, but not limited to the KAT-10 and the pre-wash tank were permit required confined spaces. On or about 11/18/14.  NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
