Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) Jobsite - On or about November 18, 2014 employee installed shingles on a 6/12 pitch roof without the use of guardrail systems, safety net systems or personal fall arrest systems. The employee was exposed to a fall height of 13 feet.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder. (a) Jobsite - On or about November 18, 2014 an extension ladder being used by an employee was not extended at least 3 feet above the 6/12 pitch roof edge. The roof edge was 13 feet above ground.
