29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: At this construction project an employee was observed standing on an inverted five gallon bucket in order to increase the working height of the platform on the second level platform of a fabricated frame scaffold.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Employees at this location were not protected from falls while working from scaffold platforms at the following areas: A. An employee was handing material from a second tier scaffold, approximately 13 feet from ground level to an employee (foreman) on a third tier scaffold without protection from falls consisting of mid and top rails, harness and lanyard, or other means of fall protection. B. An employee (foreman) was working from a sitting position at the end of the third tier high platform without fall protection
