29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, Avonlea Square @ Regency Apartments (Building #1) located at 1836 Roswell Street, Smyrna, GA 30080: Employees were exposed to fall hazards while working at heights of approximately 29-feet on a 5/12 pitched roof with no fall protection system in place.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  (a) Roof, Avonlea Square @ Regency Apartments (Building #1) located at 1836 Roswell Street, Smyrna, GA 30080: The employer did not provide fall protection training to employees working on a 5/12 pitched roof at heights of approximately 29-feet. Employees were wearing inadequate fall protection systems with missing components essential to complete fall protection systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Avonlea Square @ Regency Apartments Building #1 located at 1836 Roswell Street, Smyrna, GA 30080: CSHO observed employees using a Louisville (Model - AE2232) 32 ft. Extension Ladder which was not extended at least 3-feet past the roof landing to allow safe access off the roof. Employees using this ladder which was also not secured were exposed to falls of approximately 29-feet to the lower level.
