29 CFR 1926.304(g)(1): Radial Saws: The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will defect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.  WORKSITE: The guard for the sides of the lower blade of the DeWalt model DW705 compound miter saw, serial number 139244, was secured in the open position, exposing the lower portion of the blade.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy.   WORKSITE: The electrical box having duplex outlets, used to energize the DeWalt model DW705 compound miter saw and a battery charging unit was missing the cover, exposing the live wiring.

29 CFR 1926.451(b)(5)(i): Each end of a platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.  WORKSITE: The cantilevered portion of the scaffold planks used on the following scaffolds extended more than 12 inches, and the planks were not adequately installed and/or secured in place to prevent from tipping:                                        1.     The step ladder scaffold, the left end extended 64 inches. 2.     The step ladder scaffold, the right end extended 37 inches.  3.     The job-made supported scaffold, the right end extended 18 inches.  4.     The scaffold plank between the job-made supported scaffold and the fabricated frame scaffold, the left end extended 24 inches.  5.     The scaffold plank between the fabricated frame scaffold and another job-made supported scaffold, the right end extended more than 30 inches.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  WORKSITE: The employee, working on the 8 in 12 pitch roof as well as the top plate of the parapet wall were not protected from the hazards of fall between 10 feet 7 inches and 17 feet by the use of appropriate fall protection systems and or devices.
