J. D. Products, Inc. @ 101 Trail Ave., Troy, MO, 63379: On November 19, 2014, the employer did not ensure that an employee who worked inside a trench was equipped with head protection. The employee worked inside a trench that had excavation spoils and job materials located along the edge of the excavation. This condition exposed the employee to struck-by hazards.

J. D. Products, Inc. @ 101 Trail Ave., Troy, MO, 63379: On November 19, 2014, an employee attempted to locate a sewer tap while inside a trench that measured approximately 8 feet deep and 18 feet in length without the use of a stairway, ladder, or ramp. This condition exposed an employee to struck-by hazards.

An employee used a shovel while inside a trench to dig and locate a sewer tap. The trench measured approximately 8 feet deep where this individual was located. This condition exposed the employee to struck-by hazards.
