29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:      (a) On or about November 19th, 2014, employees operating, maintaining and servicing the Welty-Way machine (Model CR5A, Serial No. CR7504) in the coil line production area of the facility were exposed to possible injuries from unexpected start-up or machine activation in that (among other deficiencies) the lockout/tagout procedure failed to specifically identify the electrical breaker controlling power to the machine, and did not stipulate the equipment and method to be used to lockout the power source.  The "procedure" also called (inappropriately) for the application of a tag when, in fact, a lockout device is required, and did not specify the need to remove the lockout device (simply stating "Turn (the) power source back on") once work had been completed.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
