29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about September 15, 2014 on the Comco Pro Glide Press #381 machine guarding was not in place during operation of the press exposing employees to pinch points. b) On or about September 15, 2014 on the Mark Andy Press #371, machine guarding was not in place during operation of the press to allow the material to feed into the rollers exposing employees to pinch points.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments. a) On or about November 18, 2014 employees were opening the Comco #381 electrical control panel to reset the circuit switches from a three phase 480 volt main panel, were not trained in electrical safety related work practices including but not limited to 29 CFR 1910.331 through 1910.335.

29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs. a) On or about November 18, 2014 the Comco #382, 480 volt control panel was not deenergized during the resetting of switches exposing employees to live parts and/or electrical shock.
