29 CFR 1910.215(d)(3): Surface condition. All contact surfaces of wheels, blotters and flanges shall be flat and free of foreign matter. (a) On or about November 18, 2014 at the USDA Forest Service Tell City, in the warehouse/woodshop building, the surface of the grinding wheel for the 8 inch bench grinder, model JBG-8a, serial number 8100202, had extensive foreign matter imbedded in the wheel. Employees were exposed to potential struck by hazards.
