29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident within eight hours: Storage Yard at 994 Nash Street: The employer failed to report via any means, a work-related accident which occurred at approximately 1615 hours on November 18, 2014 resulting in the death on an employee. OSHA was notified of the accident by Massachusetts State Police at approximately 1200 hours on November 19, 2014.
