29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:     Plant: the employer did not adequately inspect hazardous energy control (lockout tagout) procedures periodically (at least once per year) to ensure they were being followed and that the procedures met the requirements of 29 CFR 1910.147. Employees were exposed to injury while servicing and maintaining production machinery, such as, but not limited to CNC vertical milling centers, EDM wire and EDM sinker machines.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) Production, grinding department - employees assigned to perform setup tasks in CNC creep feed grinding and CNC vertical milling machines had their finger(s) and hand(s) exposed to caught in-between and entanglement hazards capable of causing amputations where an orderly shutdown of the equipment was not carried out.      (b) Production, grinding department - employees assigned to perform adjustments in CNC creep feed grinding and CNC vertical milling machines had their finger(s) and hand(s) exposed to caught in-between and entanglement hazards capable of causing amputations where an orderly shutdown of the equipment was not carried out.  The configuration, programming and use of the access door interlock devices (alternative measures) did not provide effective protection.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    Production, grinding department: employees were operating and performing production on CNC creep feed grinders where the point-of-operation safeguarding devices were not installed and configured in accordance with applicable and appropriate standards. All hazardous point-of-operation motions did not stop. Employees opening access doors during automated process were exposed to having their finger(s) and/or hand(s) amputated.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    The Abrasive-Form, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 315239277, citation number 01, item number 003 and was affirmed as a final order on October 6, 2011, with respect to a workplace located at 454 Scott Drive, Bloomingdale, IL 60108.
