29 CFR 1910.125(f)(3)(iii): The cover with the fusible link for automatic fire prevention is not kept closed when the dip tank is not in use: In the Print Room, Crystal Clean Parts Washer 1601R, Model 54107R, Serial No. 7086716. On or about November 18, 2014, and at times prior thereto, employees were exposed to a fire hazard from a cover on the dip tank that was propped open, preventing it from closing in the event of a fire, while they worked in the area using flammable and combustible materials such as, but not limited to, Crystal Clean 142 Mineral spirits.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cable were used as a substitute for the fixed wiring of a structure or equipment: In the Laminating Room, Pullman BIV 96, Built-in vacuum. On or about November 19, 2014, and at times prior thereto, employee(s) were exposed to the hazards of a fire, from the central vacuum pump power cord being plugged into the hanging flexible Power Sentry surge Protector cord, Model 100279, instead of providing fixed wiring, while they operated and cleaned around the Multi-Cam CNC router table.
