Stoett Industries, Inc. located at 600 Defiance Ave. Hicksville, OH 43526; On or about November 20, 2014, the employer did not assure employees were protected from uncovered breaker slots in the breaker panel that powers the Screen Cutter. Employees are required to access the panel to reset the Screen Cutter and are exposed to 120/208V electrical shock hazards.
