29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. Building 5, employees were exposed to a 19 feet fall hazard, in that, while setting up rebar at the third floor employees worked at the edge of the building without any type of fall protection.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: a. Building 3, employees were exposed to a fall hazard, in that, 4 stairways consisting of two flights of stairs each used to gain access from the first to the second floor were not provided with the required handrail system to protect employees. b. Building 5, employees were exposed to a fall hazard, in that, 4 stairways consisting of two flights of stairs each used to gain access from the second to the third floor were not provided with the required handrail system to protect employees.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems: a. Building 3, employees were exposed to a 9.5 feet fall hazard from the second to the first floor, in that the 4 stairway landings at the top of each stairway were not provided with a railing system to protect employees. b. Building 5, employees were exposed to a fall hazard, in that, railings were not provided at the stairway landings of the 4 stairways leading form the second to the third floor. The four stairways had open sided landings at the exterior of the building between flights of stairs and at the top of the second flight of each stairway.
