29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    (a)  FACILITY WIDE - Employees performed daily duties to include cleaning and pool maintenance, and the employer did not ensure that employees were provided with personal protective equipment while using products, including but not limited to Muriatic Acid, Pool Lift Turbo Shock, and Ecolab Stainblaster Power Paks, thereby exposing employees to serious injuries, on or about November 7, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a)  SWIMMING POOL ROOM - Employee(s) used multiple chemicals to maintain the swimming pool, including muriatic acid, Pool Life Turbo Shock, and Sodium Hypochlorite, and there were no facilities suitable for flushing and drenching of the eyes, thereby exposing employee(s) to serious injury, on or about November 7, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
