29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section: (a) Outside Stairs: Employees climb a stairway with a width of 36 inches, that is missing a mid rail on the open side exposing employees to a fall of approximately 4ft, as observed on or about 11/20/2014. (b) Forman's Office: Employees climb a stairway to the Forman's Office which has a width of 62 inches, that is missing stair railing on the open side exposing employees to a fall of approximately 4 feet 2 inches, as observed on or about 11/20/2014. ABATEMENT CERTIFICATION REQUIRED
