At the sander department; the employer did not develop and implement a hearing conservation program for employees who were exposed to continuous noise exposures that exceeded an 8-hour time-weighted average of 85 decibels or an equivalent dose of one hundred percent.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens were not arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding.      At welding department; employees welded on stainless steel without adequate welding screens, exposing employees eye injuries.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.          Adjacent to welding department; compressed gas cylinders were not secured to prevent movement, exposing employees to struck-by hazards.
