The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury or illness. 29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition: At this location, employees were exposed to fire and explosion hazards while monitoring pressure gauges during the bleed-off of well #6. The employer allowed employees in the area where equipment not approved for the hazardous location was operated. Equipment operated in the hazardous location included equipment such as, but not limited to: A. an Allmand Light/Power Plant, Model Night-Lite Pro II. B. a Telehandler, JLG Model G9-43A. C. a Rough Terrain Telehandler, Genie Model TT-TEREX-SBFL.
