29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Across from 901 Banyard Ave:  Employees were exposed to a fall hazard of approximately 8 feet while using a ladder which was not extended 3 ft. from the upper landing.  Observed on 11/18/14   No abatement documentation or certification is required.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) Across from 901 Banyard Ave: Employer did not have certification that the employees have been trained in the proper use and operation of the fall protective system.  Observed on 11/18/14  Pursuant to 29 CFR 1903.19, the employer must submit abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
