29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: Workers were observed engaged in commercial construction activities repairing an existing sewer piping system. Workers were exposed to potential struck-by hazards in that proper means of cave-in and/or falling soil prevention was not utilized.

29 CFR 1926.651(g)(2)(i): Emergency rescue equipment, such as breathing apparatus, a safety harness and line, or a basket stretcher, was not readily available where hazardous atmospheric conditions existed or could reasonably be expected to develop during excavation activities. Workers were observed engaged in commercial construction activities repairing an existing sewer piping system. Workers were exposed to potentially hazardous breathing atmospheres and a rescue plan was not in place for the prompt rescue of workers.
