29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected: a) 1815 Forest Avenue, Staten Island, NY 10303: Employees were allowed to work constructing the roof of a building, in close proximity to floor openings at a height of 13' 4" from the ground without fall protection. Condition occurred on or about November 18, 2014.
